Mental healthcare access among resettled Syrian refugees in Leipzig, Germany

Our aim was to examine mental health needs and access to mental healthcare services among Syrian refugees in the city of Leipzig, Germany. We conducted a cross-sectional survey with Syrian refugee adults in Leipzig, Germany in 2021/2022. Outcomes included PTSD (PCL-5), depression (PHQ-9), anxiety (GAD-7) and somatic symptom (SSS-8). Descriptive, regression and effect modification analyses assessed associations between selected predictor variables and mental health service access. The sampling strategy means findings are applicable only to Syrian refugees in Leipzig. Of the 513 respondents, 18.3% had moderate/severe anxiety symptoms, 28.7% had moderate/severe depression symptoms, and 25.3% had PTSD symptoms. A total of 52.8% reported past year mental health problems, and 48.9% of those participants sought care for these problems. The most common reasons for not accessing mental healthcare services were wanting to handle the problem themselves and uncertainty about where to access services. Adjusted Poisson regression models (n = 259) found significant associations between current mental health symptoms and mental healthcare service access (RR: 1.47, 95% CI: 1.02–2.15, p = 0.041) but significance levels were not reached between somatization and trust in physicians with mental healthcare service access. Syrian refugees in Leipzig likely experience high unmet mental health needs. Community-based interventions for refugee mental health and de-stigmatization activities are needed to address these unmet needs in Leipzig.

Background
Studies have reported mixed findings regarding the impact of the coronavirus disease 2019 (COVID-19) pandemic on pregnant women and birth outcomes. This study used a quasi-experimental design to account for potential confounding by sociodemographic characteristics.
MethodsData were drawn from 16 prenatal cohorts participating in the Environmental influences on Child Health Outcomes (ECHO) program. Women exposed to the pandemic (delivered between 12 March 2020 and 30 May 2021) (n = 501) were propensity-score matched on maternal age, race and ethnicity, and child assigned sex at birth with 501 women who delivered before 11 March 2020. Participants reported on perceived stress, depressive symptoms, sedentary behavior, and emotional support during pregnancy. Infant gestational age (GA) at birth and birthweight were gathered from medical record abstraction or maternal report.
ResultsAfter adjusting for propensity matching and covariates (maternal education, public assistance, employment status, prepregnancy body mass index), results showed a small effect of pandemic exposure on shorter GA at birth, but no effect on birthweight adjusted for GA. Women who were pregnant during the pandemic reported higher levels of prenatal stress and depressive symptoms, but neither mediated the association between pandemic exposure and GA. Sedentary behavior and emotional support were each associated with prenatal stress and depressive symptoms in opposite directions, but no moderation effects were revealed.
ConclusionsThere was no strong evidence for an association between pandemic exposure and adverse birth outcomes. Furthermore, results highlight the importance of reducing maternal sedentary behavior and encouraging emotional support for optimizing maternal health regardless of pandemic conditions.
Prospective Evaluation of Factors Predicting Nicotine Withdrawal Symptoms Among Korean Americans

Introduction: This study examined factors predicting nicotine withdrawal symptoms following quitting among Korean American smokers who were receiving counseling and nicotine replacement therapy.
Methods: The sample comprised 90 Korean American smokers selected from a two-arm randomised controlled trial of a smoking cessation intervention (culturally adapted versus treatment as usual). Nicotine withdrawal symptoms were assessed weekly for the first four weeks from the target quit day, using the Minnesota Nicotine Withdrawal Scale (MNWS). Only those who participated in two or more weekly assessments of the symptoms were included.
Results: Among the nine withdrawal symptoms listed in the MNWS, craving and disturbed sleep decreased over time whereas the remaining symptoms had no significant effect of time. Women or individuals who perceived greater risks of quitting smoking reported more withdrawal symptoms after controlling for abstinence status. Although withdrawal symptoms did not change, on average, with time, the rates of change varied randomly across individuals. Women reported more withdrawal symptoms in the first week after quitting and showed a higher rate of decline of the symptoms over time than men.
Conclusions: Korean American smokers who are women or who perceive greater risks of quitting smoking may require more intensive treatment to effectively deal with post-quit withdrawal symptoms.

This article calls for a more nuanced understanding of the links between the motivations, trajectories and policy environments of community asset organisations and the geographies of their social impact. While potential for the ownership of physical assets by place-based community organisations can be found in new localism powers in all four jurisdictions of the UK, there may be differences in policy articulation and implementation that enable or limit the social benefits community asset organisations are thought to deliver. Furthermore, community assets are premised on their intrinsic tie and value to place, with social cohesion, communal mobilisation and identification of mutual interest thought to be at their heart. This article reviews research in this field set in relation to recent policy developments, and identifies an important need to better understand how the personal and social geographies of impact are delivered in, and influenced by, different spatial contexts and political frameworks.

Summary
Traditional full-thickness corneal transplantation, known as penetrating keratoplasty (PK), was first performed over a century ago and remains the gold standard of corneal transplantation. Penetrating keratoplasty involves replacement of all layers of the central 7–9 mm of the host cornea with allograft tissue, usually derived from eye banks. Donor corneas are harvested from 1–2 weeks prior to transplantation. Tissue or blood typing is not routinely done. The overall optical clarity, integrity of donor tissue, and donor endothelial cell density are evaluated at the eye bank. The tissue is screened for multiple infectious diseases of the donor, a procedure commonly performed for other human allograft tissues.
Penetrating keratoplasty
Penetrating keratoplasty usually begins with the removal of the diseased central host cornea by use of various forms of trephine. Next, the donor tissue is trephinated from the donor corneal tissue and is usually slightly oversized. The donor tissue is secured in the recipient bed using either interrupted or continuous nylon sutures. When necessary, a cataract extraction may be performed in combination with intraocular lens implantation. Monitored anesthesia care in these cases usually includes brief, intravenous sedation combined with a retrobulbar block. General anesthesia may be required for selected patients unable to be cooperative, such as children or anxious adult patients. Total surgical time is around 30–45 minutes for an experienced surgeon. Corneal graft survival at 1 year is 90% for PK. Indications for PK include haze, ectatic disease, opacities in the cornea, and corneal edema causing decreased vision or pain. In addition, infections, scars, trauma, congenital dystrophies, and corneal decompensation or injury from prior intraocular surgery are also indications.

The ‘Responsible’ Tenant and the Problem of Apathy

In the last decade, the UK New Labour government has emphasised tenant participation in housing policy. Consequently, those individuals who decide to opt out of participation processes have been problematised as ‘apathetic’, and identified as needing to be ‘empowered’ through professional interventions. Drawing on research about community ownership in Glasgow, this paper argues that tenants' reasons for not getting involved are more than simply lack of interest. Tenants articulated an instrumental approach to participation, and rejected the conflation of tenant participation with tenant management. Practical barriers also obstructed their latent motivation.
